Horizon Public Health’s Community Resource Map is an interactive tool designed to help residents quickly locate essential services and support close to home. Having access to community resources and knowing where to find them when you need them can make a meaningful difference for individuals and families during times of need.
The map connects users to a wide range of services, including:
- Health Services such as medical and dental care, hospitals and clinics, mental and behavioral health support, pharmacies, public health programs, senior services, substance use recovery resources, and vision care.
- Food Resources including grocery stores, food assistance programs, community meals, farmers markets, community gardens, and WIC services.
- Parks & Recreation opportunities such as parks, beaches, fitness and yoga centers, recreation facilities, golf, bowling, and other wellness activities.
- Child Care providers and resources to childhood support services for families.
Users can search by category or location to find resources throughout the region and connect with services that promote health, stability, and well-being.
